You can become a Life Member of the Foundation by simply completing the application below and submitting it. You can also increase your membership by becoming a Star Life (or multiple Star Life) member, or become a Bartlett Fellow or Lady also by completing the application.
 | You can become a Life Member by contributing $100. New Life Members will receive a plaque and a Life Member lapel pin. These will be sent to the Foundation Board Member for your division who will arrange a presentation. |
| You can become a Star Life Member by making an additional contribution of $100. New Star Life Members will receive a Star Life lapel pin. It will be sent to the Foundation Board Member for your division who will arrange a presentation. |
| Multiple Star Life Memberships can be made through contributions of additional $100 up to nine. Star Life Members beyond the first Star will receive a rocker to hang on their Star Life lapel pin. It will also be presented by the Board Member. |
| You can become a Bartlett Fellows and Lady once your total contributions to the Foundation reach $1,000. Additional contributions of $1,000 designate you as a Diamond Level Bartlett Fellow or Lady. Bartlett Fellows and Ladies will receive a Medallion, a Bartlett Member lapel pin, and upon request, a plaque. These will be presented to you by the Foundation President at the next District Convention or Midyear Conference. |
| You can honor a deceased individual with a Memorial to that person. Please complete the Memorial information and a card will be sent to the next of kin. |
| You can make a contribution in the name of another deserving individual. If you wish to honor another individual, you may do so by completing the application below for that individual. Fill in your name for Contributor and complete the Presentation Information area for the person being honored. |
| All contributors receive a letter of thanks from the President of the Foundation. |
The District Foundation is a 501(c) 3 organization under the IRS code and donations to it are tax deductible as a charity contribution. Be sure to consult your tax advisor about the deductibility of your contribution.
